Social Worker – Various clinic settings: Abu Dhabi / Al Ain / Al Dhafra (DCD License Holder)We are seeking x15 Social Workers (DCD licensed) to join our client with various clinic locations in Abu Dhabi, Al Ain, and Al Dhafra.Responsibilities:Assists patients, families, and significant others with their psychological, social, environmental, financial, and emotional problems by conducting psychosocial assessments related to presenting problems/concerns and providing interventions needed to enhance patient quality of living.Responsibilities• Take care of management responsibility for vulnerable clients, including production of comprehensive assessments of client needs, formulation of a social care plan, and identification of relevant resources to meet individual needs.• Coordinate delivery of social care plans and liaise with appropriate government or charity agencies where appropriate following assessment.• Carry out initial comprehensive person-centered psychosocial assessments; plan and implement individual strategies of care with the service user, care giver, and multidisciplinary team; update patient file history as required by JCI.• Act as an advocate for the patient to ensure their expressed wishes are central when planning long-term care needs.• Liaise effectively with other agencies including government and charity involved in the care of the individual to maintain and enhance professional relationships.• Work closely with patients/families to direct them to appropriate resources and support when needed.• Follow up with psychiatric visits from other hospitals to assess the patient case; record unusual conditions/limitations and document examination time and room number.• Follow up with approved ER-exempted cases of needy patients; ensure treatment coverage is processed and directed to the concerned department.• Promote awareness of social care/social work context and anti-discriminatory practice to other professionals; facilitate multidisciplinary working.• Negotiate diverse and complex needs with providers/agencies and coordinate care packages and access to resources and information.• Maintain and promote high standards of clinical record keeping, including electronic data entry, reporting, and professional self-governance per codes of practice.• Manage frequent exposure to distressed/emotional circumstances and maintain focus and concentration.• Provide crisis interventions as needed.• Participate in complex discharge planning with the multidisciplinary team to ensure smooth transitions of care for patient and family.• Participate in multidisciplinary team meetings to discuss and advocate for patient and family needs (physical, social, and emotional).• Provide clinically appropriate psychoeducation and direct therapeutic interventions to address psychosocial needs and concerns of patients and families.• Evaluate and counsel children and their caregivers on child protection issues.• Contribute to breaking bad news and the bereavement process following the facility’s processes and best practices.• Report cases of abuse or neglect of children and vulnerable individuals and document findings and responses; include vulnerable adults with financial or self-neglect issues.• Participate in social work research and stay current with literature related to practice; reflect this in practice.• Maintain confidential information about patients and staff in accordance with codes of conduct and legislation.• Ensure supervised staff comply with occupational health and safety policies and procedures.• Deliver and participate in training and continuing education sessions as required by hospital policy and DOH licensure.• Participate in department programs/committees, hospital committees, and staff meetings.• Assist and participate in orientation and training of new employees.• Ensure appropriate information sharing based on governing policies and procedures.• Maintain professional development by attending lectures, seminars, or online education units.• Complete and maintain regulatory requirements including licensure, certification, and mandatory training within established time frames.
